Nexum Injection contains Esomeprazole 40mg, a proton pump inhibitor (PPI), formulated for intravenous (IV) use. It is commonly used in patients unable to take oral medications, providing effective reduction of gastric acid secretion. Nexum is indicated in conditions such as GERD, peptic ulcers, Zollinger-Ellison syndrome, and prevention of rebleeding after endoscopic treatment of gastric or duodenal ulcers.
Active ingredient: Esomeprazole sodium (40mg per 5ml vial)
Inactive ingredients: Sterile solvent and stabilizers (as per manufacturer)

Adults:
GERD: 20–40mg once daily IV, usually up to 10 days
Zollinger-Ellison syndrome: Initial dose 40mg twice daily, then adjusted
Ulcer rebleeding prevention: Initial 80mg IV bolus followed by continuous infusion (8mg/hour) for 72 hours
Children (≥1 year): Dose based on body weight and clinical judgment
Always follow the doctor’s prescribed regimen
Symptoms: Confusion, drowsiness, tachycardia, headache, flushing
Management: Supportive treatment, monitor vital signs, no specific antidote

Common:
Headache, nausea, abdominal pain, constipation, diarrhea
Injection site pain or irritation
Serious (rare):
Severe allergic reaction (rash, swelling, difficulty breathing)
Clostridium difficile–associated diarrhea
Hypomagnesemia (low magnesium with long-term use)
Vitamin B12 deficiency with prolonged therapy
Long-term use may increase risk of fractures, hypomagnesemia, and vitamin deficiencies
Use with caution in severe liver disease
Monitor electrolytes if used long-term or with diuretics
Not recommended during pregnancy unless clearly needed
Breastfeeding: Consult doctor before use
Nelfinavir, atazanavir (antivirals): contraindicated/reduced absorption
Clopidogrel: reduced effectiveness when used with PPIs
Warfarin: may enhance anticoagulant effect, monitor INR
Methotrexate (high-dose): reduced clearance, toxicity risk
Store below 25°C, protect from light and moisture
Do not freeze
Use immediately after reconstitution/dilution
Dispose of unused vials safely, per hospital protocol
