Description

Nexum Injection contains Esomeprazole 40mg, a proton pump inhibitor (PPI), formulated for intravenous (IV) use. It is commonly used in patients unable to take oral medications, providing effective reduction of gastric acid secretion. Nexum is indicated in conditions such as GERD, peptic ulcers, Zollinger-Ellison syndrome, and prevention of rebleeding after endoscopic treatment of gastric or duodenal ulcers.

Dosage

• Adults:

  • GERD: 20–40mg once daily IV, usually up to 10 days

  • Zollinger-Ellison syndrome: Initial dose 40mg twice daily, then adjusted

  • Ulcer rebleeding prevention: Initial 80mg IV bolus followed by continuous infusion (8mg/hour) for 72 hours

• Children (≥1 year): Dose based on body weight and clinical judgment

• Always follow the doctor’s prescribed regimen

In Case of Overdose

• Symptoms: Confusion, drowsiness, tachycardia, headache, flushing

• Management: Supportive treatment, monitor vital signs, no specific antidote

Missed Dose

• As it is administered in hospital/clinical settings, missed doses are unlikely.

• If missed, the healthcare provider will reschedule the administration.

How To Use

• Administered only by a qualified healthcare professional

• Can be given as:

  • IV injection (over ≥3 minutes)

  • IV infusion (20–30 minutes or more)

• Do not self-administer

When Not to Use / Contraindications

• Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation

• Concomitant use with nelfinavir (antiviral)

• Caution in patients with severe liver impairment

Side Effects

Common:

• Headache, nausea, abdominal pain, constipation, diarrhea

• Injection site pain or irritation

Serious (rare):

• Severe allergic reaction (rash, swelling, difficulty breathing)

• Clostridium difficile–associated diarrhea

• Hypomagnesemia (low magnesium with long-term use)

• Vitamin B12 deficiency with prolonged therapy

Precautions & Warnings

• Long-term use may increase risk of fractures, hypomagnesemia, and vitamin deficiencies

• Use with caution in severe liver disease

• Monitor electrolytes if used long-term or with diuretics

• Not recommended during pregnancy unless clearly needed

• Breastfeeding: Consult doctor before use

Drug Interactions

• Nelfinavir, atazanavir (antivirals): contraindicated/reduced absorption

• Clopidogrel: reduced effectiveness when used with PPIs

• Warfarin: may enhance anticoagulant effect, monitor INR

• Methotrexate (high-dose): reduced clearance, toxicity risk
