Clexane is a low molecular weight heparin (LMWH) containing enoxaparin sodium, supplied in sterile pre-filled syringes. It is an anticoagulant used to prevent and treat blood clots in conditions such as deep vein thrombosis (DVT), pulmonary embolism (PE), and certain heart conditions. Each pre-filled syringe contains 60 mg enoxaparin in 0.6 mL solution for subcutaneous injection.

Treatment of acute DVT and PE (with or without warfarin)
Management of unstable angina and non-ST elevation myocardial infarction (NSTEMI)
Prevention of clot formation in patients undergoing hemodialysis
Prevention of DVT: 40 mg SC once daily; in higher risk patients, 30 mg SC every 12 hours.
Treatment of DVT/PE: 1 mg/kg SC every 12 hours OR 1.5 mg/kg SC once daily.
Unstable angina/NSTEMI: 1 mg/kg SC every 12 hours.
Hemodialysis: 1 mg/kg into the arterial line at the start of dialysis.
Elderly/renal impairment: Dose adjustment required.
Route: Subcutaneous injection (do not give IM).
Overdose may cause excessive bleeding.
Management: Stop enoxaparin, administer protamine sulfate (partial reversal), provide supportive care.
Immediate medical attention required.
Take as soon as remembered if not too close to next scheduled dose.
Do not double dose.
Consult healthcare provider for guidance.
Wash hands and prepare injection site (abdomen, at least 5 cm from navel).
Remove needle cap, do not expel air bubble from syringe.
Pinch fold of skin, insert needle vertically into skin fold.
Inject entire contents slowly.
Do not rub injection site.
Dispose syringe in sharps container.
Active major bleeding
Severe thrombocytopenia
Known hypersensitivity to enoxaparin, heparin, or pork products
History of heparin-induced thrombocytopenia (HIT)
Do not administer intramuscularly
Common:
Pain, redness, bruising at injection site
Mild bleeding (nosebleeds, gums)
Elevated liver enzymes
Serious (seek medical help):
Severe bleeding (internal or external)
Heparin-induced thrombocytopenia (HIT)
Allergic reactions (rash, swelling, difficulty breathing)
Spinal/epidural hematomas (if used with spinal anesthesia)
Use under medical supervision only.
Increased risk of bleeding with other anticoagulants or antiplatelet drugs.
Caution in kidney impairment, elderly patients, or low body weight.
Avoid intramuscular injections.
Do not self-administer without training.
Special caution during spinal/epidural procedures (risk of hematoma).
Increased bleeding risk with:
Anticoagulants (warfarin, dabigatran, rivaroxaban)
Antiplatelets (aspirin, clopidogrel, ticagrelor)
NSAIDs (ibuprofen, diclofenac)
Thrombolytics (alteplase, streptokinase)
Store below 25 °C, protected from light.
Do not freeze.
Keep out of reach of children.
Use pre-filled syringe once; discard after use.
Dispose of needles/syringes in sharps container.
