Description

Humalog Mix-25 is a pre-mixed insulin preparation that combines:

  • 25% Insulin Lispro Injection (rapid-acting)

  • 75% Insulin Lispro Protamine Suspension (intermediate-acting)

This mix provides fast onset of action to control blood sugar spikes after meals and longer duration of action to maintain basal control. Supplied in convenient KwikPen pre-filled pens, it is easy to use for self-administration in people with diabetes.


Ingredients

  • Active ingredients:

    • Insulin Lispro (25%) – rapid-acting

    • Insulin Lispro Protamine (75%) – intermediate-acting

  • Excipients: glycerol, protamine sulfate, zinc oxide, phenol, metacresol, water for injection, hydrochloric acid/sodium hydroxide (pH adjuster)

Uses / Indications

  • Management of Type 1 Diabetes Mellitus

  • Management of Type 2 Diabetes Mellitus (when oral medicines are inadequate)

Dosage (general guidance, individualized by physician)

  • Adults and children: dose is individualized based on blood glucose monitoring, dietary intake, and activity level.

  • Typically injected immediately before a meal, but may also be taken just after eating if necessary.

  • Dose adjustments required during illness, stress, or changes in diet/exercise.

  • Always follow physician’s instructions on dose timing and adjustments.


In Case of Overdose

  • May cause severe hypoglycemia (low blood sugar).

  • Symptoms: sweating, trembling, palpitations, hunger, confusion, fainting, or seizures.

  • Treatment: oral glucose (if alert), or emergency IV glucose/glucagon injection (if unconscious).


Missed Dose

  • If a dose is missed, check blood glucose immediately.

  • Inject the next scheduled dose at mealtime; do not double inject to compensate.

  • Consult doctor if multiple doses are missed.


How To Use

  1. Wash hands before handling the pen.

  2. Gently roll the pen between palms to resuspend the cloudy insulin (do not shake vigorously).

  3. Attach a new needle and prime the pen before each injection.

  4. Inject subcutaneously into the abdomen, thigh, buttock, or upper arm.

  5. Rotate injection sites to avoid lipodystrophy (skin changes).

  6. Store pens in refrigerator before opening; in-use pens can be kept at room temperature (below 30°C) for up to 28 days.


When Not to Use (Contraindications)

  • Hypoglycemia (do not inject if blood sugar is already low)

  • Hypersensitivity to insulin lispro or any excipients

  • Use with caution in patients with renal or hepatic impairment (dose adjustments may be needed)


Side Effects

Common:

  • Hypoglycemia (most frequent, potentially serious)

  • Local injection site reactions (pain, redness, swelling)

  • Weight gain

Uncommon:

  • Lipodystrophy at injection site (lumps or thinning of skin)

  • Edema (fluid retention)

  • Mild allergic skin reactions

Rare/serious:

  • Severe allergic reaction (anaphylaxis – very rare)


Precautions & Warnings

  • Monitor blood sugar regularly with self-testing devices

  • Adjust insulin dose during illness, surgery, or stress as advised by doctor

  • Be cautious while driving or operating machinery if prone to hypoglycemia

  • Avoid alcohol, which increases risk of low blood sugar

  • Safe during pregnancy and breastfeeding under medical supervision, but dose adjustments are required


Drug Interactions

  • May increase risk of hypoglycemia with: oral antidiabetic agents, ACE inhibitors, MAO inhibitors, salicylates, sulfonamides, alcohol

  • May reduce insulin effect with: corticosteroids, oral contraceptives, diuretics, thyroid hormones, sympathomimetics

  • Always inform healthcare provider about all medicines being used


Storage / Disposal

  • Store unused pens in refrigerator (2–8°C). Do not freeze.

  • In-use pens: keep at room temperature (<30°C) for up to 28 days.

  • Protect from direct heat and sunlight.

  • Dispose of used pens and needles safely in sharps containers.
