Description

Humalog Mix-50 is a premixed human insulin analog containing 50% insulin lispro (rapid-acting) and 50% insulin lispro protamine suspension (intermediate-acting). It provides both rapid onset and intermediate duration of action, making it suitable for mealtime glucose control and basal coverage. Supplied in a KwikPen pre-filled device for convenient, accurate subcutaneous injection.


Ingredients

  • Active Ingredients:

    • Insulin lispro (soluble, rapid-acting) – 50%

    • Insulin lispro protamine (suspension, intermediate-acting) – 50%

  • Excipients: Zinc oxide, glycerol, phenol, m-cresol, protamine sulfate, dibasic sodium phosphate, water for injection, etc.

Uses / Indications

  • Treatment of Type 1 diabetes mellitus

  • Treatment of Type 2 diabetes mellitus when oral agents are insufficient

  • Provides both prandial (mealtime) and basal insulin coverage in a single injection


Dosage

  • Individualized according to patient’s blood glucose, diet, activity, and medical advice

  • Usually injected within 15 minutes before a meal

  • May be administered once or multiple times daily depending on regimen

  • Total daily insulin requirement generally 0.5 – 1.0 IU/kg body weight/day (combined with other insulins if needed)

  • Dose adjustments required during illness, stress, surgery, or pregnancy


In Case of Overdose

  • Main risk: severe hypoglycemia (sweating, tremor, hunger, palpitations, confusion, seizures, unconsciousness)

  • Management:

    • Mild: consume fast-acting carbohydrates (glucose tablets, fruit juice)

    • Severe: require glucagon injection or IV glucose in a medical setting

  • Continuous monitoring is essential until stable


Missed Dose

  • If missed and close to mealtime, inject as soon as possible

  • If next scheduled dose is near, skip missed dose and resume as normal

  • Never double the next dose to compensate


How To Use / Administration Instructions

  1. Wash hands and prepare injection site (abdomen, thigh, upper arm, or buttock).

  2. Prime the pen as per manufacturer’s instructions.

  3. Dial the prescribed dose.

  4. Inject subcutaneously; do not inject intramuscularly or intravenously.

  5. Rotate injection sites to prevent lipodystrophy.

  6. Store the pen in use at room temperature; do not refrigerate once opened.


When Not to Use / Contraindications

  • During episodes of hypoglycemia

  • Known hypersensitivity to insulin lispro or any excipients

  • Not for intravenous use


Side Effects / Adverse Reactions

Common:

  • Hypoglycemia

  • Injection site reactions (pain, redness, swelling, itching)

Less common:

  • Lipodystrophy (lipoatrophy or lipohypertrophy)

  • Allergic skin reactions

  • Edema (fluid retention)

Rare but serious:

  • Systemic allergy/anaphylaxis

  • Severe hypoglycemia leading to coma or seizures


Precautions & Warnings

  • Monitor blood glucose regularly

  • Adjust dose with changes in diet, activity, or illness

  • Use with caution in patients with renal or hepatic impairment

  • Insulin requirements may change during pregnancy and lactation

  • Carry glucose source at all times to treat hypoglycemia

  • Alcohol may enhance risk of hypoglycemia


Drug Interactions

  • Increase hypoglycemic effect: oral antidiabetic drugs, ACE inhibitors, beta-blockers, salicylates, sulfonamides, alcohol

  • Decrease effect (raise glucose): corticosteroids, oral contraceptives, thyroid hormones, diuretics, sympathomimetics

  • Beta-blockers: may mask symptoms of hypoglycemia (except sweating)


Storage / Disposal

  • Store unopened pens in refrigerator (2°C – 8°C); do not freeze

  • Once in use, store at room temperature (<30°C) for up to 28 days

  • Protect from heat and direct light

  • Dispose of pens and needles in appropriate sharps container
